Formatting your sick leave application-Attached Samples!
A leave application is a letter written to pertain for a leave of absence from your services for a specific time duration. Just as the name suggests sick leave is basically a leave of absence due to illness. Working on the days when we feel physically or mentally unhealthy is unpreferable as it reduces productivity. Therefore, the employee has every right to take a few days off to get back to his/her normal condition. Through this letter, the employer is being informed about the unavailability of the employee from the workforce. So a sick leave letter must be written in a professional manner, although there are no formal guidelines to write a sick leave application. Still, you can use the following guidelines to help you write a formal sick leave application.
Check the sick leave policy of your corporation.
Before writing the letter the first step is to know about the sick leave policy of the company. You can find this from the company’s handbook or by asking your senior/colleagues in case of any doubt. Nowadays every organization has its unique leave portal where they can mark their absence in case they need leaves for 2,3 days. However, if an employee requires leaves for a large number of days, they are required to submit an application.
Recipient
The recipient is the initial component of the letter. The recipient is the reporting manager to whom your letter is being conveyed. He/she is the one responsible for the approval/rejection of your letter.
The subject of the Sick leave Application
The subject is the central component of your leave. It must be precise and clear. A long or vague subject can result in delaying or getting your recipient confused It is the first thing to be read by the recipient. It should be like `Leave application for a week` or use any other effective way that conveys the straight message.
Greetings
Always use a greeting before writing the main context of your letter. It makes your letter put together and well structured. Moreover, greeting shows a sense of respect and empathy. Here are some instances: “Dear Sir/Mam” or “Dear Mr./Ms.“.
The context of your Sick Application
The exact reason for your leave should be presented appropriately. Simply stating that you need a leave without giving proper reason will be unacceptable. Be descriptive yet precise in giving reason. You can also attach the medication given by the doctor. Moreover, Medical certificate will help you to approve your sick leave soon.
Stipulate number of days
Don’t forget to mention the number of days you will be absent from work. You can also mention particular days like from the 12th of November till the 18th of November or by simply mentioning a weak or specific number of days starting from the day of leave initiation.
Don’t forget to mention your work/health balance
Remember to offer assurance to your boss that your work will not be affected in any way by your absence. You can assign someone else from your office to look after or simply by asking a favor from your colleague to take care of your important tasks for the time being. Mention, that you will be putting up some extra efforts after your leaves to make up for your work imbalance due to your sickness.
Complimentary ending
Always use some complimentary text like
- Kindly approve my leave starting from [Start day] to [End day]
- Best wishes
- Close your statement with “Yours sincerely/truly” “Warm Regards“
- Your credentials.
Don’t forget to mention your contact information in case your employee wants to contact you for an emergency. You should mention the ideal way in which your boss can communicate with you.
Attach medical proofs
If you have mentioned above about the medical documents then don’t skip to attach the files at the end. It could be a diagnosis, doctor’s prescription, medical certificate, or a note. However, don’t pile up your attachments, keep them at minimal number.
Proofreading
Before sending the leave application, give a quick read just to check if there is any date error or typos. Check out the leave dates properly. As there will be no correction if sent.
>>So now you can write a sick leave in a professional manner without giving much thought on it. Follow the above points while writing a sick leave and you will be good to go. Date
Subject: Leave application for a week
Hello Jonathan
I am writing this letter to inform you of a medical issue which renders me unable to attend the office. For the past three days, I have been suffering from a severe migraine attack and my doctor has advised me a week’s complete bed rest to soothe my nerves which are causing constriction pain in my head and continuous nausea.
I am sorry to apply for leave during this busy season. On the other hand, I had completed the report I promised to deliver and will email the same to you shortly for your review. After my leaves, I will put in extra effort to patch up the pending work.
I will be very thankful if you will grant me a week’s leave from November 21st, 2019. Thank you in advance for your consideration.
Yours sincerely,
